Medical Form

A medical form is required for camping and other outdoor activities.  We highly recommend having this form completed at your Scouts annual physical - it makes it easy to ensure you have it when you decide to participate in high-adventure activity.  

Please note - this form is VERY important to have filled out correctly.  If your Scout has any allergies or known existing medical conditions, please make sure they can explain to the Scoutmaster.
